§ 5-115. Filing and contents of application.

Any person desiring a permit required by this division shall make application therefor to the chief of police on a form prescribed by him, which application shall state:

(1)

The name of the club and the location where the same will be maintained, conducted or operated.

(2)

The name and address of all persons interested in the ownership thereof, if privately owned, and the names and addresses of all officers and trustees thereof, if owned by a nonincorporated association.

(3)

The purpose for which such club is organized.

(4)

The name and address of the person who will be in actual charge of the operation of the club.

(5)

Such other pertinent information as the chief of police may require.
